Admissions and Discharge

Admitting Information

Hospital admitting is available on a 24-hour basis, but please follow your physician's advice as to actual time of admittance. Financial arrangements should be made with our credit manager at 209.944.5550, ext. 3114, prior to admission.


It is necessary that you sign the Conditions of Admission Form, which includes an authorization for medical and surgical treatment. You will also be asked to sign personal financial liability and assignment of insurance benefits forms.


It is important for us to know about the prescription and over-the-counter medications you are presently taking, such as vitamins, herbal supplements, etc. Please fill out the attached admissions form to help us collect that information.


If you are to have surgery or undergo special procedures, the nurse will ask you to sign a form for authorization and consent to surgery or special diagnostic or therapeutic procedures. Parents must sign forms on admission for children under 18 years of age. The admitting clerk will notify you when your room is ready, and you will be escorted to your room.


If you are scheduled for surgery, please view the attached admissions forms. These forms are to be filled out and mailed to us prior to your preoperative appointment. When mailing, please be sure to send to the attention of the Preadmissions Department.


Discharge Information

Discharges from the hospital should be completed prior to 11 a.m., so please make your arrangements for transportation accordingly. Your doctor will tell you when you are ready to go home from the hospital. Your nurse will discuss your discharge instructions, and you will be escorted by our staff to your car.


Life Line

Life Line—an in-home, voice-to-voice, 24-hour communications system offering security and a human link to those who are living alone or ill—is available to discharged patients. With a touch of a call-button on the unit or on a wearable pendant, the confined person can be in instant contact with someone who can help in an emergency.
